Aadhaar Card – Enrolment, Download, Status, UIDAI Services

• Full Name:
  Provide your complete name, including first name, middle name, and last name. It should match the name on your identity proof.
  
  
•  Gender:
  Indicate your gender as male, female, or transgender on the form.
  
  
• Age/Date of Birth:
  Enter your date of birth in the format DD/MM/YYYY. If you are unsure of the exact date, you can approximate your age. If there is no proof of date of birth, select the ‘Declared’ option. If you have supporting documents, select the ‘Verified’ option.
  
  
• Address:
  Fill in your postal address as mentioned in your address proof. Remember that the Aadhaar card will be delivered to this address. Also, provide your email address and mobile number in the same field.
  
  
•  Details of Family Members:
  Include the names and Aadhaar numbers of your family members, such as your father, mother, husband, or wife. For children below 5 years, it is mandatory to provide the details of parents/guardian.
  
  
• Consent:
  Tick the box to give UIDAI permission to share your information, including biometrics, for Aadhaar generation and authentication with relevant agencies providing public/welfare services.
  
  
• Documents Attached:
  Tick the appropriate box based on your application type. Mention the documents you are submitting, such as proof of identity and proof of address. If applicable, include proof of relationship and proof of date of birth. If you have mentioned an introducer/HoF, the documents can be verified from their file.
  
  
• Signature:
  Sign the document in the designated area on the form.

 Aadhaar card correction form must be submitted for any changes to be made to your Aadhaar.

The following details must be filled in the Aadhaar Card correction form for rectification:

•  Pre-enrolment ID
  
  
• NPR number
  
  
• Name of the applicant
  
  
• Gender – Male, Female, Transgender
  
  
•  Age and Date of Birth (DoB)
  
  
•  Complete residential address, Email ID provided previously, and registered contact number
  
  
•  Data about father/mother/guardian/husband/wife. Mandatory for children below 5 years of age.
  
  
•  Details of supporting document proofs – proof of identity, residential address, DoB
  
  
•  Data about Introducer or HUF (Hindu United family), if required
  
  
• Signature or thumbprint of the applicant.
