Assessing Officer Code:
Fill in the Assessing Officer (AO Code) details, including AO Type, Range number, or AO number, as available on the NSDL website.

Provide your title (Mr./Mrs./Ms.), surname, full name, and middle name. (Use the full expanded name as per proof of identity/address documents; initials are not permitted.)
Other Name:
If applicable, mention any other names you have been known by.
Gender:
Tick the appropriate gender option; applicable only for individual applicants.
Date of Birth:
Enter your Date of Birth/Incorporation/Agreement/Partnership/Association of Person.
Details of Parents:
Enter details for father’s name (mandatory) and mother’s name (optional).
Address:
Fill in both residence and office addresses with detailed information.
Address for Communication:
Indicate the preferred address for communication (residence or office).
Telephone Number and Email Address:
Provide a mobile number and valid email address for regular updates on your PAN application.
Status of the Applicant:
Tick the appropriate status (Individual/HUF/Company/Partnership Firm, etc.).
Registration Number:
Mandatory for companies, firms, etc.
Country of Citizenship and ISD Code:
Mention your country of citizenship and its ISD code.
Source of Income:
Fill in the source of income, including Business or Profession Code if applicable.
Representative or Agent in India:
Provide the full name and address of the representative or agent in India. Names should not be in abbreviations.
Documents Required:
Specify the name of the document submitted as Proof of Identity (POI) and Proof of Address (POA).
KYC Details:
